We ask that every prospective client complete
an Intake and Cast of
Characters & Chronology Form upon arrival to an appointment. As an
added
convenience, we have provided the forms for download from our web site. You may
also complete these forms online using our secure, encrypted forms. Filling out the form prior to your arrival will shorten the amount of time
spent for the initial consultation and will ensure a more productive one as
well.
The downloadable forms are saved as Microsoft Word files. Word files are
cross platform meaning they can be read by both IBM-PC and Apple-Macintosh
systems.
To download a form, simply click on the form listed below that best
describes your legal needs. Your computer will ask you where you wish to
save the file on your system. Navigate to the folder you wish to save the
file in and click "SAVE".
If Microsoft Word doesn't automatically open upon completion of the download
process, open Microsoft Word manually. Select Open under the file
menu and
locate the file you've downloaded.
All three files are saved with the file name beginning with "HLO". To
find the file you downloaded look for a file with that text in the name.
